About National Well Home Network
Our mission is to foster Housing-Based Service Models (HBSMs) across the country. With a network of HBSMs in every state, we can create one, integrated system of supports for seniors and individuals with a disability. HBSMs are defined by eight key components. Developing Housing-Based Service Models requires partnerships at all levels with funders, national organizations and local community-based organizations. The evidence is compelling and growing.

Housing-Based Service Models
HBSMs are person-directed service delivery systems that use housing as a platform to integrate health and social services. Participants benefit from a seamless set of services that include care management, care coordination, transitional care, and facilitated self-management of chronic conditions, medication management, health education, and wellness services.
SASH Proven Results
Health and housing partnerships have resulted in reduced utilization of emergency departments, hospitals, nursing homes, and rehab facilities. HBSMs and other interventions have reduced spending in Medicare and Medicaid. The statewide Support And Services at Home model in Vermont has reduced the rate of Medicare spending by $1,227 per person per year among the early cohort participants (read more). We can only achieve our vision working in partnership with community-based housing and social service agencies, hospitals, primary care practices, accountable care organizations, payers and all levels of government. We partner on research, programs and education with NeighborWorks, Enterprise Community Partners and LeadingAge. The National Well Home Network is funded by Wells Fargo Housing Foundation established in 1850—re-established in 2018.
